AFFIDAVIT OF LOSS

I, MARIA LUNA, Filipino, of legal age, married and a resident of Tacloban City,

having been duly sworn to in accordance with law do hereby depose and state that:

1. I am the President of Luna Foundadtion Incorporated;

2. I represent the company Luna Foundation Incorporated which is a bonafide

member of the Philippine Business Bank (PBB), and as such, was issued one (1)

PBB Passbook with account number of 000-00-000000-0;

3. As the company president I had custody of the said passbook;

4. Around the second week of October 2015 I had my assistant do a general

cleaning in my office;

5. During such cleaning, some files and documents had been thrown out and I only

realized that the above mentioned passbook was inserted into one of the folders

that my assistant got rid of;

6. Upon knowing such fact, my assistant and I immediately started looking for it

exerting such efforts and diligence in locating the same but all efforts turned

fruitless;

7. The same item was not confiscated for violation of any law or ordinance nor

conveyed or assigned for value to any person; and

8. I have executed this affidavit to attest to the truth of the foregoing facts and for

whatever legal purpose the same may serve.
