Adapted Equipment Available for RentalHaving a party or special event? Recreational and adaptive equipment willnow be made available to individuals residing in any of our eleven partnerdistricts for a fee of $5.00 per item. Equipment that may be rented includesan adaptive bowling ball and ramp, parachute, pillo polo set, and a cageballwith a colorful canvas cover. Dial 711The relay service works by connecting calls between a speech or hearingimpaired person using a text telephone, or TTY, and a hearing person usinga regular telephone. The person using the TTY types his or her conversationand the message relayed to the other party by a communications assistant(CA). “It Only Takes A Village or Two”Is a new support group for Leyden Township area families who havechildren on the autism spectrum. Teacher’s & classroom aides are welcome,too. The group works to support each other and to learn more about theimpact of autism on children and their families. Illinois Special Olympics provides year round sports training in a variety of Olympic-type sports for children andadults with mental impairments or closely related developmental disabilities who are 8 years of age or older. Theprogram gives athletes opportunities to develop physical fitness, demonstrate courage, experience joy and participatein the sharing of skills and friendship with their families and other athletes. WSSRA supports the mission of SpecialOlympics through the participation in the following sports: * Track & Field * Softball * Aquatics * Basketball * Bowling *Volleyball * Golf *

CRITERIA FOR PARTICIPATION

All Special Olympic athletes must have a current, completed Application for Participation in Illinois Special Olympics form on file withthe Special Olympic coordinator at WSSRA.This form contains four sections:

1. Athlete Information 2. Parent and/or Guardian Authorization & Medical Authorization2. Health Insurance and Emergency Information 3. Medical Clearance (This MUST be completed by a physician).

Applications for Participation in Illinois Special Olympics are valid for 2 years based on the earliest signed date on the medicalapplication form. One of WSSRA’s very own was selected to represent Illinois at theSpecial Olympic Nationals held in Lincoln Nebraska in July. Frank Pattersona resident of Oak Park and a long time participant with WSSRA wasselected to swim four events at the National games. Frank is a top

athlete in most every sport heparticipates in. He especially excelsat swimming and due to his consistentperformances he was selected. It isquite an honor to be selected and atremendous accomplishment toachieve a gold medal at these events.Frank won his gold by finishing firstin his division in the 100 yard Freestylerace. Frank will be honored at theupcoming WSSRA Sports banquet onSunday, September 19. Frank is thesecond WSSRA athlete to go toNationals. Georgia Hunter was ourfirst.

A new and more affordable swimming option. Parents, now you will have anopportunity to watch your child improve an already learned stroke, learn newstrokes and feel comfortable in the water while floating independently. This isa WSSRA Skill Development program. All skills will be taught in a progression,and a progress report will be given at the conclusion of the season. Bring suitand towel. Parents are responsible for changing child before and after lesson.Participants are limited to one half-hour lesson per season. Lessons aredesigned to be appropriate for specific age groups as well as skilllevel.PLEASE NOTE: ANYONE WITH POSSIBLE INCONTINENCE MUST WEARPROTECTIVE PANTS UNDERNEATH THEIR SWIMSUITS, This is for everyone’shealth & safety. NON-DISCRIMINATIONThe West Suburban Special RecreationAssociation (WSSRA) is aware of the Americanswith Disability Act (ADA) which prohibitsdiscrimination on the basis of disability. CLOSED PROGRAMSIf a program closes because we have reachedmaximum enrollment, you will be put on a waitinglist and contacted if additional space is available.

REGISTRATIONAny cancellations for fall programs will beaccepted through September 13, 2010 andcredit will be issued. Any cancellations afterSeptember 13, 2010 participants will becharged full program fee. Any unpaid balancewill restrict participation in futureprograms.

REFUNDSRefunds will only be issued when the registrantcannot participate for extended medical reasonsand/or hospitalization verified by a physician. Forthe majority of the season the refund/credit willbe prorated. Call for more information.847.455.2100.

PAYMENT POLICYPayment of program fees is due upon registrationunless other arrangements are made. Statementsare mailed out the first week of the programseason (4 times per year). If changes are madein the registration (adding programs, cancellingprograms) please call the office for your accountbalance as no other statement will be sent.Callthe Finance Manager to arrange a payment plan,Monday through Thursday, 8:30 AM to noon. Aparticipant cannot register for the nextseason until balance from previous seasonis paid.

RETURNED CHECK POLICYA $20.00 fee will be charged for any checksreturned to WSSRA. The $20.00 fee, as well asthe balance, will need to be paid before registeringfor future programs.

SCHOLARSHIP REQUESTWSSRA is in a position to offer scholarships toWSSRA residents for a percentage of fees fortwo programs each season. Persons requestinga scholarship must complete a scholarshipapplication, which will be mailed to you uponrequest. Scholarship applications need to be filledout once a year. If you are interested in ascholarship, check the box on the registrationform: Request Scholarship. All scholarshipapplications must be completed and in the WSSRAoffice by September 13, 2010

NON-RESIDENT POLICYWSSRA programs are limited to people who livein the WSSRA member districts and village. If youlive outside the WSSRA communities and areinterested in participating in a special recreationprogram, call your park district or villagerecreation department. Call the WSSRA OutreachSpecialists for more information.

WSSRASKILL DEVELOPMENT PROGRAMS

Due to popular demand, we have selectedspecific programs that will focus on progressivedevelopment of specific skills. Each skill will begradually introduced, based upon the participant’sability level. At the end of each session, eachparticipant will receive a progress report. WSSRAwill keep these reports on file for our use inplanning additional programs. In keeping with ouroverall philosophy, we will stress enjoyment ofthe program by participants. Recreation for thefun of it remains our number one objective!

SAFETY PRECAUTIONSSome participants in WSSRA programs andspecial events may be carriers of infectiousdisease(s). Staff and volunteers have beenprovided inservice training concerning properhygienic procedures. WSSRA seeks to provide asafe environment for all of our participants, staffvolunteers and the public. If you have any specificquestions, please call the WSSRA ExecutiveDirector or Superintendent of Safety &Operations at 847.455.2100.

INSURANCEThe Association takes every reasonableprecaution to prevent injuries and accidents fromoccurring. However, if an accident does occuror if you or your child is injured, you are solelyresponsible for any costs incurred or caused byan injury that happens in an Association program.

MEDICATIONIt is the parent/guardian and participant’sresponsibility to inform WSSRA of any medicationa participant is taking. This information is veryimportant in case of an emergency. Medicationinformation should be noted on the registrationforms, as well as the Annual Information Form.Any changes in medication should becommunicated to WSSRA.

PARTICIPANT ILLNESSFor the protection of all the participants inprograms, you/your child must be kept at home orwill be sent home if he/she shows any of thefollowing symptoms:♦ A temperature of over 100.3.♦ Stomach ache accompanied by diarrhea

or vomiting.♦ Any undiagnosed rash.♦ Sore or discharging eyes or ears.♦ Profuse nasal discharge (green or yellow).

Please keep children with active coldshome.

♦ Have a highly contagious condition suchas chicken pox, measles, lice, etc.

REVISEDLATE PICK-UP/DROP-OFF POLICY

In order to be fair to our participants, programstaff and drivers, WSSRA has enacted a LatePick-up/Drop-off Policy. Many of our program staffand drivers need to get to a program immediatelyafter the program your child is in. We ask thatyou make sure you are on time picking up yourchild from program, as well as home to greetyour child when the WSSRA van drops them off.

A $10.00 fee will be charged for every 15 minutesor portion thereof, a participant is picked up late,or a van is delayed, after a 10 minute grace period.A notice will be sent to you indicating the feebeing charged to your account.

Also, participants must be dropped off on timefor programs that are traveling, including overnight trips. Vans will not wait more than 5 minutesfor participants who are late for the program.

Saturday morning programs - Parentsmust drop off their child by 9:30 AM or call theemergency phone. If the program is traveling, thevan will not wait longer than 5 minutes past theprogram start time.

PARTICIPANT ABSENCEIf you or your child are not going to attend arecreation program that you are registered for,please notify WSSRA office 847.455.2100 ASSOON AS POSSIBLE! Please leave a message onVoice Mail after hours.

CANCELLED PROGRAMSA. If four participants or less register, a program

may be cancelled. A program reimbursementor credit will be issued.

B. If a program is cancelled because ofunforeseen circumstances you will becontacted at the soonest possible time.

GOOD SPORTSMANSHIPWSSRA encourages parents and families toattend games and tournaments and cheer theteens/young adults on! We expect coaches,players and parents to demonstrate goodsportsmanship during these events. This includesrespect and support of coaching staff andofficials who are working on behalf of yourplayer. Any person not demonstrating goodsportsmanship including behavior outburststowards a player or coaching staff and/orofficials, that is deemed threatening, will not betolerated. You may be asked to leave and yourfuture participation and attendance at events willbe in jeopardy. By registering your son/daughter/dependent for these programs you are agreeingto follow these guidelines as written. Please referany concerns or questions to the Superintendentof Recreation. We look forward to a safe andsuccessful season.

WSSRA TRANSPORTATION1. Door to door transportation is available for all programs noted with the

transportation symbol. This extra service is available if you pay a$6.05 per round trip fee, for each day the program meets.

2. Transportation to programs from designated pick up points is availablefor a fee of $2.75 per round trip, for each day the program meets. Youmust pay at time of registration.

3. All participants being transported in WSSRA vehicles must abide byWSSRA guidelines. Failure to do so will result in suspension fromtransportation. Repeat offenders will not be transported by WSSRA.

4. Parents or participants must notify the Association in regardto any change in transportation.

5. If an Association vehicle cannot complete a route and family membersmust pick up riders, a credit will be issued for the participant’s lateruse.

6. Any registration received after September 13, 2010 may result in alate program start or no transportation.

7. Parents are requested to notify their child’s school on days studentsare being picked up by a WSSRA van.

8. A participant under the age of 16 must be taken off the van by aresponsible person over 12 years old.

PLAYING IT SAFEDue to new state legislation, carseats or booster seats must be providedfor all children ages seven and under. For more information contact BobFoster.

WAIVER OF LIABILITYThe national liability crisis the past few years has been severe for agencieslike WSSRA. Our insurance premiums used to be $800 a year and nowexceed $17,000 a year. To help keep the price of insurance low and tobetter manage our risks, we have joined a self-insurance pool with otherspecial recreation associations and park districts.

Our pool is known as the Park District Risk Management Agency (PDRMA).As a loss prevention measure, PDRMA is requiring participants to executea release of liability for programs. The reverse side of the registrationforms contains the WSSRA waiver release form. This must be signed andreturned to the WSSRA office before participants can be in programs.

Since we do not carry medical or accident insurance for programparticipants, the release of liability is necessary. The cost of such coveragewould make our program fees too high for most to afford. Please reviewyour personal health plan to be certain that you and your family havecoverage in instances like these.

Our objective is to offer a high quality program at a reasonable cost. Theuse of a release of liability when appropriate is but one way we are tryingto meet that objective. If you have any questions, call Sandra Gbur at847.455.2100. We appreciate your continued involvement and support.

WALK-IN OR MAIL-IN OR FAXEDREGISTRATION PROCEDURES

3. All signed registration forms received by September 13, 2010 willbe processed by the WSSRA Registrar and approved for programand transportation information.

4. All registrations will be accepted on a first-come, first served basisuntil we reach our maximum enrollment for each program.

As a participant in these programs, I recognize and acknowledgethat there are certain risks of physical injury, and I agree to assumethe full risk of any injuries, damages or loss which I may sustainas a result of participating in any manner, in any and all activitiesconnected with or associated with such activities and involvesubstantial risks of injury.
