Gerken will have flaggers to assist in directing both vehicle and pedestrian traffic along with Caution tape and constructionbarrels as required.

For a portion of the time the main pedestrian walk way along the fence will be closed and an alternate route will be required.

Please do not cross the caution tape as it is there for your safety.

SAFETY REVIEW BOARD March 27, 2019 FINGER INJURY REDUCTION INITIATIVE Reviewed a finger injury reduc-tion initiative – safety tour. - In 2018 GM had 9 permanent disabling injuries. Seven of these were finger-tip amputations. Of 17629 total injuries in GM, 2130 were finger injuries (12%). - In 2018, Defiance had zero permanent disabling injuries. Of 100 total injuries in Defiance, 12 were finger injuries (12%).- Objective: Conduct focused safety tours and safety conversation tours to identify and control hazards that can potentially cause finger injuries. The goal is elimination of permanent disabling injuries.

We want everyone that comes on to our property to be able to return home safely. - Reviewed the results of SCM program. o2016: 639 total incidents, 245 recordables, 4 permanent disabling injuries, 1 fatalityo2017: 555 total incidents, 190 recordables, 3 permanent disabling injuries, 3 fatalitieso2018: 263 total incidents, 114 recordables, 4 permanent disabling injuries 0 fatalities.- Reviewed Defiance Contract Managers. If you observe a contrac-tor doing something that is not safe, the first thing you should do is stop them. We will report it. The grade will change. We do an eval-uation to drop the grade.PS20 Respiratory Protection. Reviewed the current overview:- Highlights of Performance Standard: oEveryone needs to be trained annually on dangers of Silica and the OSHA Appendix D -the voluntary use of respirators.oEveryone that is required to wear a respirator for any part of their job has to be fit tested and use the proper respirators that they are fit tested to.oRespirators have to be stored in a clean dry place and be main-tained per the manufacture’s recommendations. -Evidence of impl. for all departments and all shifts: oThe training review did show some employees that were red. These employees need caught up on their respirator training. -Areas of improvement/upcoming changes for next year There were changes to the PS on March 1, 2019. These changes drove 3 of the 4 CAP’s that were written.- Review corrective action plan:o4 CAP’s were written to shore up short falls from updates to the PS and working on CI within this program.CAP TRACKER REVIEW Past due action plans. - PS19 – Physical Agents Assessment and Control. - PS1 – Confined Space. Have 2 open items.

- 2.1 – Identification of Reg. Requirements & GM Standards. There are 19 on list. Will assign dates and owners by 4/5/19. - 2.2 – Hazard & Risk Identification, Assessment & Control. Have 2 open items.- 3.6 – Safe Operating Practices. Have 1 open item.- 3.7 – Emergency Response Plan. Have 1 open item.Will review this monthlySENTINEL EVENT SUMMARY Reviewed Sentinel Event summary. 18-00039 – Make-up air units- 19-00018 – Hatch: Sponseller is working on a design for a fall hazard rail system. Due date is 5/15/19. 19-00035 – Roof doesn’t have guarding. 19-00036 – Roof outside the Training Center. Plan is to install new temporary hand rail around exposed roof by 5/31/19. 19-00037 – Manhole cover: Found a fall hazard device. Have over 100 hatches and have 74 manholes. Target date is 5/31/19.19-00038 Fall hazard. This is weirs in the pond. Installed anchorage point and fabricated adapter for drill on weirs. Working on guard rail design. SPRAINS & STRAINS New Sprains & Strains committee - 8 total strains (2 recordable strains) Slipped and fell on ice at turnstile and Strained right knee – Employees knee locked up. 63% of our 2019 CY strains have been the result of slips/falls. Reviewed plan for strain reduction.Stress the injury prevention program. During the past year, Tyler has 1100 interactions with employees talk-ing about nutrition, injury prevention programStrain categories Push/pull, repetitiveness, slip/trip, inadequate/faulty equipment/awareness. Change tools/work practices if applicable. - Utilize SafeStart Ergo and Strains/Sprains module learningsNOISE COMMITTEE Reviewed updates to hearing protection maps- Waste Water Treatment plant: Hearing protection not required. -Central Utilities Complex: Hearing protection required.- Plant 2 East: Hearing protection required.- Plant 2 West: Hearing protection not required.- Plant 1: Premachining is being retested on March 29. -Pit in Water Treatment plant required to wear hearing protection. FOLLOW-UP - Reviewed the probation letter for lockout and the scorecard. At 92%. Have 46 people left to complete. - Reviewed a safety incident from Bedford. A Team of Die Setters were tasked with removing a die from a Die Cast machine. They locked out the 5 required energy sources and began the removal. The activity began on 3rd shift and continued into day shift. Since the process required multiple shifts, a departmental transition lock was placed on the group key box by the die setters group leader. During day shift activities, an electrician was assigned to a separate task of removing an ancillary piece of equipment. The electrician locked out the disconnect to the ancillary equipment, placed the key inside the standard group key box and placed his personal lock on the group key box. Upon completion of the task, the electrician contacted his group leader, confirmed that the transition lock was adequate and removed his personal lock. Later that shift, the die set-ter team returned to the machine to power it up and reposition the tooling for removal. The die setter group leader removed the transi-tion lock, the employees removed their personal locks and all 6 en-ergy sources were powered up. The die setter team did not realize that they had inadvertently energized the exposed conductors to the now removed ancillary equipment. Two shifts later, the ancillary equipment was ready to be re-installed. As the unit was being trans-ported to the area, the electrician noticed the exposed conductors lying on the ground. The electrician elected to kick the conduit which contained the wires out of his way instead of picking them up with his hands. The live conductors arced upon contact with each other. Reviewed the initial action required by all sites.-Reviewed Line 6 Decommission status. We have snow fencing on order for 3 areas – FN-11 Oscillators area, Line 6 Sand System area and Line 6 Cope & Drag area. Reviewed the 1 open item from PIV. oComplete install of truck gate crossing arms. Baker Shindler to in-stall. The target date is 3/31/19. Should be completed this weekend.-Reviewed Wentzville Sentinel Event with fork truck falling into pit. Looking for areas where fork truck drive over open pits or grating. ROUNDTABLEFollow-up from Commercial Group on hooks. As long as the eye on the hook and the lifting bail is perpendicular to what you are lifting it does not matter whether the opening on the hook faces towards the installer or the material that is being lifted. It would be best practice to keep the open bail on the hook towards the installer. There is a possibility the tip of hook get caugh. Would like to see the hooks out.
