be turned in by August 31, 2017. Complete one form per child. Please fill out a medical statement as well. At OLOS, we understand that parents are the primary teachers of the faith. We are here to assist and support you in your instruction through our catechetical program. According to the Catechism of the Catholic Church,

• "Parents have the first responsibility for the education of their children." (CCC 2223) • "The role of the parents in education is of such importance that it is almost impossible to provide an

adequate substitute." (CCC 2221) It is a privilege and a joy for us to join you in the religious education of your children. Our Lady of Sorrows Parish School of Religion desires to enhance the efforts of families by sharing with children the sacred scriptures and the faith found in the Roman Catholic tradition. It is our hope that our students will gain a deeper appreciation of the Mass and grow further in their relationship with Christ. PSR classes do not and should not take the place of regular Mass attendance. The Eucharist is the “source and summit if all Christian life”. Mass is the most important component of your child’s faith formation.
