Havelock, Sept 05: Just whenthe Union Minister of State forHome Affairs, Mr Kiren Rijijuwas at Havelock Island, aMajor Fire Broke out at DiveIndia’s Kitchen and Restau-rant.

The Fire was re-ported at 2.30 am that too viaVHF from Port Blair. The net-work at Havelock was downand hence the communica-tion could not be passed to Fire

EDITORIAL.....

About three months ago, I was asked totalk about the biography of the founder-chairman of the group of hospitals I workin. This was a day before its official re-lease. The biographer was a noted colum-nist of Newsweek and some importantwestern dailies. I believe, one of the rea-sons of my being there was, that the chair-man having related his story in so manyinterviews and queries, himself wanted toknow how he had been caricatured, andhow an ‘odd one out’ like me would inter-pret it. He is a man, as some of us call him,of multiple and tremendous instincts!

As I heard myself speak, beforethe hospital faculty, the chairman and thecelebrated biographer beside me on the dais,on an important biography read overnight,I realized I was speaking a truth I wasnever aware of. I said that this book wasnot a book on Dr Zhivago, not on DrKotnis, certainly not on the Mother Teresamission. I was quite clear that the frame-work of working in making this big a hos-pital chain was more like a biography ofDonald Trump, Richard Branson, HenryFord. Medicine, I realized, had to run some-where close to the principles of an enter-prise, Yes, the way the biographerchronicled it, it was all about getting a gov-ernment sanction, bidding on a piece ofland where luckily the tender opened fa-vorably on the amount, beyond whichthere was no money in the kitty. It wasabout allocation of cement and steel andexcise concessions on medical equipmentcoming for the first time in the country. Itwas about knocking the door of every jointsecretary concerned, and arranging for guar-antees for loans from banks.The concept of ‘corporatization’ of medi-cine was budding in what was euphemisti-cally termed a ‘developing’ country, a bitmore than three decades ago, when Indianpresidents would line up on the appoint-ment lists of the celebrated Prof DentonCooley! No need to mention lesser mor-tals. They were perceptibly mortal! Cer-tainly, controversies were taking rounds ifrunning medicine as an enterprise was away of making money out of others’ mis-eries! I would not be surprised if that in-deed was the major issue.

This chain of hospitals grew tobe the largest in Asia in three decades. Itwas a template for many others to follow.The message I heard myself reason andlisten to was that ‘corporatization’, (mean-ing public shareholding, listing on the stockexchange), was perhaps one viable method,among others, to make a hospital run in‘profits’. A textile mill may fall in debt, amajor studio may close down, but a hos-pital closing down due to losses is a socialcatastrophe. In today’s economics, thatmakes a lot of sense.I am writing this inthe backdrop of popular measures beingannounced in changing quite a few prac-tices that have ensnared the health sys-tems in the country — the resurgent, andrather appropriate urge to ‘clean up’ thesystem. Nothing could be more appropri-ate and timely. I have no doubt that thecommon man should never be cheated, andthe rich man never give up his wisdom andpatience. That’s how society, systems,and nations run. The Indian health struc-ture, pharma included, has largely growndespite the government, rather than apolicy-based governance. Beyond the laud-able ‘firsts’, as AIIMS, PGIs, the initialstate-run medical colleges, the system didnot catch up with the growing needs ofhealthcare, an unbridled populationgrowth, and the threshold changes in medi-cal science. In this circumstance, privateinitiatives had to set their camps. Somewere well-regulated, where there were wellstructured by well meaning industrial

houses, corporate hospital chains as I havehad some insight into, and nursing homesof repute run by senior faculty attached toteaching institutions.

Along with this came, much tothe respite of physicians and surgeons inmid and even small towns, not to mentionthe metros, near adequate private investi-gative infrastructures as CT scan machines,MRIs, nuclear scanning, and pathologicalwork-up labs that can deliver some intri-cate tests in some of the remotest cornersof this country in a span of a day or two.This was service as I would see it first,and to run it there came a business likestructure. Why the governments could notinstall and maintain such facilities, for atleast half of the population, is a questionthat probably has easy answers. One maysay the government did not have enoughfunds, or that private participation wasbelieved to be more competitive to the con-sumer, or for other reasons one may notlike to state.

I am aware all patients, all doc-tors have to resort to support from thiseasily accessible infrastructure, that isprompt, has competitive rates. The rea-sons are again rather obvious. To take ashot at oneself, there could be doctors’interest, perhaps these investigation cen-ters announce packages to the patients,and patients of their own declare their in-tentions to avail themselves of that. Quiteoften, there are third parties employed thatmay influence a patient, which could be afamily kin that has a holding, and becomea decision maker. Despite this, I am readyto agree that some methods have gonemuch beyond control. To sum up the sum,money transactions, whether it be currencyconversion, credit card usage, ticketing, allrun on mutual or even benefits across thechain. Call it the grease and oiling, call itmuck.

The one essential message hereis that in a flight of righteous chauvinism,regulating agencies have to realize that thissystem still delivers, though in many waysis now becoming destructively unethical.There should be a well-thought policy howto replace the practices methodically bywhat are fair business practices that areviable and more importantly acceptable tothe common man that he is not beingcheated. It’s like refuelling a plane midair.One has to catch up to the speed andspace, and there should be no jolt whilethe process is on. The clean up may in-clude transparency, definition of what areacceptable practices, deterrence where itshould be used, and activating government-run centers, again by private participationif that be so. The wisdom of bringing aboutchanges in a system that grew on its ownto provide services, that thrived more be-cause of need and subsequently runs ongreed, will require mostly common sense,deciding what are reasonable charges forsuch services, and defining what may becalled as ‘crossing the limits’, knowing justthat slight difference of what makes thehorse run, and what are the advantages, ifany, of flogging a tired horse.

With further opening of markets,investments, particularly FDI in insurance,there is a need to improve systems by allmeans, even by providing more medicalbenefits and social security to the com-mon man, but so is the need to keep theIndian medical houses healthy enough tocompete with any foreign players. Thecorrective measures need not flog them intodormancy; on the contrary make themcleaner and more competitive. It is a subtlewisdom, how to keep your house intact,even while replacing what has become un-wanted. Because it is your house, and youlive in it.

Heart Experiment aLesson For All: Docs

BANGALORE: A day after aheart from a brain-dead patientin Bangalore was successfullytransplanted to a patient inChennai, doctors in the city saidthe whole exercise demon-strated that proper coordinationbetween various agencies in-volved in emergency servicescan save lives.“The whole pro-cess -- right from the planningstage --- will be documented sothat we can create awarenessabout how a well-planned andcoordinated effort can help,”Dr N K Venkataramana, ViceChairman and Neuro Surgeon,BGS Global Hospitals, told Ex-press. The operation was con-ducted at BGS and police hadcreated green corridor to en-sure that the cadaver heartreached the airport without anydelay and flown to Chennai.

B Dayananda, Addi-tional Commissioner of Police,Traffic and Security, said,“The traffic department ishappy that the whole operationbecame a success due to itsefforts and coordination be-tween stakeholders.”“Effortsare on to adopt the technologywhere a signal will turn greenwhen an ambulance is ap-proaching a signal. The budgetallocated is ̀ 72 crore and by theend of this financial year, wewill be able to roll out theproject,” said Dayanand.

Immediately after wegot information from the hos-pital about a brain dead case at4.40 pm on September 2, werushed to the hospital and man-aged to convince the parentsbut the ball was in thehusband’s court. We managedto get their written consent,”said Manju.The Procedure DrVenkataramana said the heartwas stopped through a preser-vative solution and kept in aclean box. This procedure wasdone last as the in-house teamof surgeons from BGS Globalfirst removed the liver and kid-neys. The heart was retrievedlast as it had to be transportedwithin six hours. “The heart inthis procedure has to be kept insub zero temperature and oncea recipient is found, the arteriesand veins are connected andslowly the temperature is in-creased to reactivate it,” said DrVenkataramana. L Sathish, Co-ordinator, Fortis Malar Hospital,Adyar, Chennai, said there wasno complication and the patientspoke to family members anddoctors after surgery. He is stillon ventilator and may be shiftedto the special ward soon.

Island Teacher Gets National AwardPort Blair, Sept 05: Mr.Sadananda Halder,PST(Bangla), of Govt.SeniorSecondary School,Pahalgaon, Mayabunder hasbeen informed by the Govt.of India, Ministry of Humanresource Development , De-partment of School Education& Literacy, Shastri Bhavan,New Delhi 110115, vide let-ter No.D.O.No.F.i-57/2014-NAT that he has been se-lected for the NationalAward,2013 under theMHRD’s Scheme of NationalAward to teachers. Hon’blePresident of India shall give

Cautioning againstovercrowding in the envi-ronmentally sensitive Is-lands of Andaman &Nicobar, Mr Rijiju said con-gestion would hamper itsecological protection andsustainable development.

He favoured effec-tive regulations in allowingpeople from outside the Is-lands in restricted areas.

However, he empha-sized that while protectingthe cultural identities andlivelihoods of the indig-enous population, promo-tional activities should alsogo hand in hand in such away that balanced growth isensured without compro-mising on the interests of thetribal people. He said theGovernment has proposed

to give 33 percent reserva-tion to women in policeforce, and it would come intoforce very soon.

He stressed the citi-zens to be gender-sensitiveand to respect the rights anddignity of women.

Under police mod-ernization programme, moreclosed circuit TVs and bet-ter street lighting and trafficsystems would be providedapart from augmenting thestrength of police personnel,Mr Rijiju said.

“Actions are beinginitiated at a fast pace toovercome the problem ofpoor telecom connectivity inthe Islands that suffer frompaucity of satellite transpon-ders.

Rs 150 crore has al-ready been released to theIslands Administration bythe Centre for improvingtelecom connectivity,” he in-formed.

The Chief Secretaryof the Andaman & NicobarAdministration, Mr AnandPrakash and the Member ofParliament, Mr Bishnu PadaRay were also present.
